1. Completed work equal to a standard high school course in Chemistry with satisfactory achievement (grade "C" or better) within the last five years.
2. Completed health form including health history, physical exam, an immunization record. Clinical experience will not be permitted without a completed health form.
3. As needed for clinical participation:
   1. CPR certification,
   2. Annual urine drug screening
   3. Current criminal record check, and
   4. Child Abuse Clearance
4. Completed application for Admission and a non-refundable $30.00 fee; payable once.
5. Official High School or GED transcript.
6. Official college transcript from all college/universities attended, if applicable
7. Three letters of recommendation.
8. SAT I or ACT college board scores if you have been out of school less than five years. The ACT is offered monthly at Mount Aloysius College.
   1. An SAT score of 900 or above (or comparable ACT score) is a prerequisite for the program.
   2. Registration for the Institutional Placement Exam (IPE), to be administered at DRMC. To register for the IPE please contact the Center for Lifelong Learning office.
