The transcript is Mount Aloysius College's official statement of your academic record.

Transcript Request Form - Click to download form in PDF format. Please print, complete transcript request form and return to the Registrar's Office by mail, fax, or in person.

If you are away from the college campus, you can mail or FAX a written request to the Registrar's Office. The request must contain the following information:

  • Student's Name (include any previous names from time student began coursework at the College)
  • Student Social Security number
  • Date of Birth
  • Dates of attendance at Mount Aloysius College
  • Your return address and a daytime phone number where you can be reached
  • The number of transcripts requested
  • Name, title, and address of person to whom each transcript is to be sent
  • When having multiple transcripts mailed to you, indicate if they should be placed in separate, sealed envelopes.
  • The student's handwritten signature and date MUST appear on the request
  • Enclose a check for $5.00 for each transcript, payable to Mount Aloysius College
  • Transcripts are not issued until all outstanding accounts with the College are paid.
  • Through compliance with federal regulations regarding confidentiality of educational records, the Registrar's Office WILL NOT process requests made over the telephone, by e-mail, or by anyone other than the student whose transcript is being requested.

    Transcripts will be mailed out within five (5) business days from the date the request is received. Processing time after the end of each semester could increase by several business days due to the high volume of requests received. Standard delivery of transcripts is by first class U.S. mail.
