Fundraiser Approval Form

Only one on-campus fundraiser will be permitted per day.

Funraiser initial requests will only be accepted at the Mandatory President & Treasurer training, September 1 at 3:30 or 4:30 p.m. (choose one)

Fundraisers will be approved on a first come, first serve basis.

This form must be submitted to the Director of Student Activities before permission for the fundraiser can be given.

    Do Not Forget, this is only to approve of your fundraiser. This is not a reservation for the space or a table. You must contact Shelley Campbell at 814-886-6335 and complete a facility request form, which can be found on the portal to reserve a space and table.
